Documents Required for Three-Wheeler License:

1. Age Proof (any one):

   • Birth Certificate

   • PAN Card

   • Passport

   • 10th Marksheet

2. Address Proof (any one):

   • Aadhaar Card

   • Voter ID

   • Passport

   • Utility Bill (Electricity/Water)

   • Rental Agreement

3. Identity Proof (any one):

   • Aadhaar Card

   • PAN Card

   • Passport

   • Voter ID

4. Learner’s License (Mandatory)

5. Form 4  Application for Permanent Driving License

6. Form 1A  Medical Certificate (compulsory for transport/commercial license or if age > 40)

7. Passport Size Photographs

8. Driving Training Certificate (from a registered driving school ? Form 5, for commercial license)

9. Fee Receipt Proof of payment
