1. Applicant’s Identity Proof

   • PAN Card, Aadhaar Card, Passport, or Driving License (Individual/Firm/Company)

2. Business Proof

   • GST Registration, MSME Certificate, Shop Act License, or Incorporation Certificate

   • Partnership Deed (for partnership firms)

3. Address Proof of Applicant

   • Electricity Bill, Telephone Bill, or Bank Statement (recent copy)

4. Trademark Details

   • Logo (if any) in JPEG/PNG format

   • Wordmark or Slogan (text-based trademark)

   • Description of goods/services

5. Trademark Class

   • Selection of correct class(es) as per NICE Classification

6. Power of Attorney (Form TM-48)

   • Signed by the applicant to authorize a trademark attorney/agent

7. User Affidavit (if trademark already in use)

   • With date of first use, along with proof (e.g., invoices, ads, packaging)
